Background

On July 10th 2015, a large cosmetic factory was on fire due to a leak on a conveyor tube. Twenty eight victims died on site, 31 patients suffer from burn and 20 of the most severe patients were immediately brought to our center. This paper intend to document the inhospital care of these patient as a single-center descriptive study.

Methods

We conducted a retrospective study, gaining information mainly from medical records. We recorded all patients from submission until end of treatment.

Result

Those patient’s TBSA range from 12-76%, all came with inhalation trauma. Intubation was done at the emergency room. TBSA was recalculated on emergency room and resuscitation fluid was administered immediately.

Nine patients were transported to our burn unit, while 11 was transported to general high care and intensive care unit.

Over the first week, 30 operations were performed,  2 patients underwent CVVH, 3 patients were extubated (all with TBSA <30%).  Of all 20 patients, the mortality rate was 60% (mostly due to sepsis).  After 68 days of multidiciplinary care from our burn center, 8 patients has been discharged with routine visit to rehabilitation clinic.

Conclusion

This is the first massive burn incident (involving more than 10 patients) encountered by our center. Adequate preparation and early resuscitation was performed and therefore suceeded to achieve a survival rate of 95% within the first week. There are needs of improvement in multidisciplinary approach and local resources for our center to meet the requirements of treating mass burn casualty. Nevertheless, the incident has enriched us with experience and a big homework to improve our burn center.
